Registering Your Child

If you are new to the School District of Philadelphia or did not complete the previous year in the School District of Philadelphia, please use this link to register your child to the School District of Philadelphia. Please note that this does not guarantee placement into Mastbaum AVTS!

Upon acceptance to Mastbaum, the following forms must be completed to properly enroll your child:

  • EH-40
  • Release of Information (FERPA)
  • S865: Student Health Status
  • Emergency Contact Information
